Absence Documentation
If you miss class due to attending an appointment here at the Health Center, we can provide a proof of visit note for you to submit to the Dean of Students through their Absence Documentation Form.
You may also submit a photo of a positive take-home COVID test (with your TCU ID in the picture) to the same form.
